Recall Information
Bausch + Lomb is conducting a voluntary recall of its PreserVision® Eye Vitamin AREDS 2 Formula with Omega 3 soft gels (SG). We are recalling our product after a small number of customers predominately within the age range of 70 years and older reporting difficulty swallowing and/or a choking sensation when taking the soft gel.
The formulation for PreserVision® Eye Vitamin AREDS 2 Formula with Omega 3 SG is safe. This voluntary recall is limited only to the United States as the product is only available in U.S. retail stores.
Bausch + Lomb closely monitors customer feedback on all of our products; it is critically important that consumers have a product with which they are comfortable using daily. Our customers are our top priority. Many of our customers are able to swallow the pill without any issues. Despite this we are requesting that consumers return all unused to Bausch + Lomb by contacting our customer service centers (numbers detailed below).

Packaging and Dosage: PreserVision Eye Vitamin AREDS 2 Formula with Omega 3 Soft Gels are packaged in 60 count bottles. The current dosing is two soft gels per day.
The following products are NOT affected by this voluntary recall:
Ocuvite®, Ocuvite® Extra, Ocuvite® with Lutein, Ocuvite® Adult 50+, PreserVision® AREDS SG and tablets, PreserVision® Lutein SG.
Product Return Procedure for Consumers
Consumers who have affected product should return it to Bausch + Lomb by calling customer service.

Return to Bausch + Lomb: We will provide consumers with a pre-paid shipping envelope with an address label to return the product to us with or without receipt.
Reimbursement: Bausch + Lomb will reimburse consumers who have purchased the product for the full price of the product. Bausch + Lomb is also providing affected consumers with a coupon for a free bottle of any PreserVision® Eye Vitamin OTC supplement; effectively giving consumers a bottle of their choice from the PreserVision® product line, free of charge.
Coupon Disbursement: A coupon for free PreserVision® Eye Vitamin supplement will be sent to qualified consumers with delivery approximately within three to four weeks of customer inquiry.
What if I have unused product, do I have to return it or may I just throw it away?
